Our Work

Supporting The Most Vulnerable

At the onset of the crisis, we moved quickly to meet the health and basic needs of those who couldn’t shelter in place.

Our work with Health Homes and Pathways to Health + Home continues to operate normally to ensure individuals experiencing homelessness and/or have complex health issues can still get the health, behavioral health and housing support they need.

Our team is in the field, on the ground, every day to:

  • Continue working with clients in achieving long-term housing stability
  • Coordinate e-consult telehealth for unsheltered clients in need of care
  • Support with SSI claims/applications
  • Deliver supplies to homeless service providers
  • And provide safety provisions to homeless encampments

Community Kitchen Project

In an innovative approach to prevent an outbreak among the most vulnerable in the community, we partnered with Solomon’s Delicatessen, who initially closed down because of the pandemic. Our partnership transformed the local restaurant into a community kitchen to make and deliver meals daily to our most medically fragile clients — both unsheltered and newly housed.

Within 10 weeks of the project, we delivered over 21,000 fresh meals. This Community Kitchen Project was made possible by our generous donors:
