Data-Driven Approach

We document, manage and share information using multiple data platforms to support cross-sector collaboration, integration of programs, and improved quality and efficiency of care.

Integration and Collaboration

Guided by our mission, we work with public and private programs and systems to integrate care that aligns with the needs of the client. We are leaders in creating data reporting and sharing that help to uniquely combine health, behavioral health and social services.

Our Care Management Platform — a population health data infrastructure that we built and manage — supports documentation of goals and outcomes, service and referral tracking, collaboration among all our team members, and reporting and analytics. The secure platform helps to identify gaps in care, reduce duplication of services and improve programs and client outcomes.

Shared Care Plan

In our work with the City of Sacramento’s Whole Person Care program, we hosted the Shared Care Plan portal – a centralized database used by all partnering agencies to communicate, share information and co-design care plans for individuals experiencing homelessness.

The portal allowed for data mining and analysis to drive services, improve cross-system care coordination activities, and improve health and housing outcomes.

Sacramento Community Network

Working with Unite Us to host our Sacramento Community Network, this collaborative referral system helps to improve clients’ overall wellbeing and address social determinants of health. Agencies that are linked to a client can view their client’s referral journey, including current status and updates on services provided.

Since our launch in April 2020, we continue to see development in new partner relationships, an increase in users, and expansion of the platform throughout the Greater Sacramento Region.

My Navigator Resource List

Our My Navigator Resource List is built on the Find Help platform and serves as a referral and community resource tool to address social determinants of health. Through this online tool, any community member can find preventative and primary care, wellness programs, food, childcare, and other resources. Straight from their device.
