Everyone should have a place to call home and access to comprehensive health care. For those at-risk of or currently experiencing homelessness, access to customized care and housing stability is essential. This not only improves the quality of life for these individuals but improves the quality of life in the community as a whole. Access to stable housing is essential to improving overall health.

Health and Homelessness

As a partner of California's Health Homes Program and also a leading partner of the City of Sacramento's Pathways to Health + Home initiative, we help provide customized care and housing stability to improve the lives of those experiencing homelessness and complex health needs. We work with numerous comprehensive health care, housing, and social service partners to build an integrated system of Whole Person Care.

Whole Person Care

Whole Person Care ensures a "no wrong door" approach that stabilizes and connects those experiencing homelessness to the right care in the most appropriate setting. This increases our community's capacity to service and deliver a more responsive system of care.

Sacramento Covered's Role

Utilizing a community health worker approach, we employ assertive outreach and assist with eligibility and enrollment into programs. We also coordinate care across organizations and systems, host and manage the care management platform, and collect data from outcome tracking.
