Knowing where to go to receive healthcare services can get confusing. We work alongside hospitals in the Sacramento region to support patients visiting the emergency department for non-urgent, primary care reasons and connect patients to a primary care home for their wellness, prevention, chronic care, and other health needs.

in emergency departments readmission rates for nonurgent, primary care reasons. We empower patients with the connections and tools they need to self-manage their health and stay safe, post-hospital discharge.

Since 2013, our patient navigators have facilitated over 32,000 clinic visits for emergency department patients seen for primary care reasons. Our patient navigators provide comprehensive follow-up to schedule medical appointments for primary and specialty care, and mitigate specific barriers to health, including English as a second language, transportation and housing.

Continuity of Care

We aim to keep people healthy and help them overcome barriers that may lead to added hospitalization. Our patient navigators work closely with hospital staff, providers, health plans and patients. They provide patient advocacy, care coordination, and education to help patients improve self-care and knowledge of the healthcare delivery system.
