Access to primary care is essential to reducing health disparities in our communities. We support patients visiting hospital emergency departments for non-urgent, primary care reasons and provide them with the necessary intervention to reduce barriers to timely treatment and care.
We work alongside hospitals throughout the Sacramento region to help patients get established with a medical home for all their health needs, including their wellness and chronic care. We provide comprehensive follow-up to help patients get primary and specialty care and mitigate specific barriers to health, including English as a second language, transportation and housing.

In the last year, our patient navigators facilitated over 32,000 clinic visits for emergency department patients seen for primary care reasons.
Continuity of Care
We aim to keep people healthy and help them overcome barriers that may lead to added hospitalization. Our patient navigators work closely with hospital staff, providers, health plans and patients. They provide patient advocacy, care coordination, and education to help patients improve self-care and knowledge of the healthcare delivery system.
